Patients with a terminal illness and life expectancy of 6 months or less
Live in the metro Atlanta area
Open to people of all faiths and backgrounds (Jewish hospice with inclusive care)
Application Process
Call 404-351-1897 to speak with a care coordinator.
A physician referral is usually required.
The team will conduct an evaluation and create a personalized care plan.
Care can often begin quickly once eligibility is confirmed.
Payment Information:
Covered 100% by Medicare, Medicaid, and most private insurance with no copays for hospice-related services.
Free of charge for uninsured patients through charitable support.
Description
Weinstein Hospice provides compassionate end-of-life hospice care in the home setting. Services include nursing, physician care, pain and symptom management, social work, spiritual care (including rabbinical support), and bereavement services. It is Atlanta’s only Jewish hospice but welcomes patients and families of all faiths or no faith.
