Your appointment is a service covered by Georgia Medicaid
You live in a Georgia county served by Verida (includes Cobb, Fulton, DeKalb, Gwinnett, Cherokee, Douglas, Forsyth, and many others -- call to confirm your county)
Application Process
Call Verida at least 3 business days before your appointment
Have ready: your Medicaid number, date of birth, address, emergency contact, doctor's name, appointment address, type of appointment, and any special needs (wheelchair, oxygen, escort)
You may schedule up to 30 business days in advance

Description
Verida provides free rides to medical appointments for Georgia Medicaid members who have no other way to get there. Verida arranges the right type of vehicle for your needs -- this could be a regular car, a wheelchair van, a minibus, or a stretcher van. Rides must be booked at least 3 business days ahead of your appointment. One adult escort or attendant may ride with you at no charge if needed for medical reasons -- must be requested when scheduling. Shared-ride service; you may ride with other passengers.
