Who Qualifies
Please reach out to the organization to determine eligibility. A referral for hospice evaluation and treatment from a Licensed Care Provider is required.
Application Process
Call 770-969-8354 and ask to speak with the Director of Social Services, Director of Nursing, or the charge nurse.
The team will collect health information and explain services.
A doctor’s referral or hospice certification (such as a Certification of Terminal Illness) and current health records are needed to evaluate eligibility.
If appropriate, an on-site evaluation and care planning meeting is scheduled.
Description
The Hospice Program at Southwest Christian Care offers end-of-life care that focuses on comfort, dignity, and quality of life for individuals facing a life-limiting illness. Care is provided in a warm, home-like environment, and staff refer to those they care for as guests. The hospice team helps with pain and symptom management and supports emotional and spiritual needs — not only for the guest but also for their family. Care is given with compassion, respect, and a focus on meaningful time together.
