Application Process

1. Call 404-688-1350 to schedule an appointment or register as a new patient.

2. New patients can also walk in during open hours, but calling ahead is recommended.

3. Bring: Photo ID, proof of address, proof of income (for sliding fee scale), and insurance card (if you have one).

4. A financial counselor will help determine your eligibility for discounts.
