Who Qualifies

To qualify:

You must live in the United States
Have a valid prescription for an eligible Abbott medication or medical nutrition product from a U.S.-licensed doctor
Meet household income eligibility guidelines (contact the program for details)
Have no health insurance or limited insurance coverage that does not adequately cover the prescribed product

Application Process

Download the form here. Fill out the Patient Information section yourself. Check “Uninsured” if you have no insurance.
Attach proof of income (tax form, paystub, or Social Security letter).
Have your doctor, SLP, or dietitian complete the medical sections
Both you and your provider must sign and date the form.
