Who Qualifies

To be eligible for admission, an applicant must meet all of the following:

1. Diagnosis of Incurable Cancer
Must provide documented medical evidence of incurable cancer.

2. Financial Need
The applicant must be financially unable to afford nursing care elsewhere, defined as having:

  • No insurance coverage

  • Inadequate insurance to cover nursing care costs

  • No other assets that would cover the cost of nursing care

3. Informed Consent for Palliative Care
The applicant or responsible party must understand that the care is palliative (comfort-focused, not curative) and accept this philosophy.

4. Valid DNR Order
A Do Not Resuscitate (DNR) order must be on file prior to admission.

Application Process

1. Obtain and complete the application form located on this website.

The applicant or responsible party must complete the formal application and pre-admission form.

The completed forms must be sent by fax to the Home at (404) 588-9568. Do not email sensitive health information.

2. Provide Required Medical Documentation
Attach documented proof of a diagnosis of incurable cancer (e.g., pathology report, CAT scan, biopsy report, or similar records).

All treatments must be completed before acceptance; the Home provides only palliative (comfort) care.

3. Submit a Valid DNR Order
Because the Home only provides palliative care for terminal illness, a valid Do Not Resuscitate (DNR) order must be submitted before admission.

3. Review and Assessment
The Home’s staff reviews the application and supporting documentation. They may contact the physician or the responsible party for clarification as part of the review process.
