Call 678-374-5134 on Monday mornings between 10:00 AM – 12:00 PM to inquire about new patient openings.
Provide proof of Rockdale County residency, proof of income, and proof of no insurance.
Once approved, you will be scheduled for an appointment. Space is limited and there may be a waiting list.
