Uninsured or underinsured individuals and families
Low-income patients
They serve patients from 26 Northeast Georgia counties and some bordering areas in North and South Carolina
Application Process
Call the location closest to you or the main office at 706-788-3234.
Ask to schedule an appointment or register as a new patient.
Bring: Photo ID, proof of address, proof of income (for sliding fee scale), and insurance card (if you have one).
A staff member will help with registration and determine your fees.
