Who Qualifies
Patients with a terminal illness and a life expectancy of 6 months or less (certified by a physician)
Live in one of the counties below:
Hall, Barrow, Jackson, Banks, Franklin, Habersham, Hart, Lumpkin, Rabun, Stephens, and White counties.
They may also serve some neighboring counties (Dawson, Forsyth, etc.) on a case-by-case basis.
Application Process
Call 770-219-8888 to speak with an Access Center Nurse.
A physician referral is usually required (the hospice team can assist with obtaining one).
The team will conduct an evaluation and coordinate care (home or inpatient).
Payment Information:
- Covered 100% by Medicare, Medicaid, and most private insurance with no copays for hospice-related services.
- Free of charge for uninsured patients.
Description
Heart of Georgia Hospice provides comprehensive end-of-life hospice care, including symptom management, pain control, emotional and spiritual support, and bereavement services for patients and their families. They offer both home hospice and inpatient hospice care.
