Who Qualifies
Terminally ill patients with a life expectancy of 6 months or less (as certified by a physician)
Patients and families residing in the Middle Georgia service area (primarily Houston, Peach, Bibb, Crawford, and surrounding counties)
No income or insurance requirements — care is provided regardless of ability to pay
Application Process
Call 478-953-5161 during office hours to speak with admissions.
A physician referral is usually required (or the hospice team can assist with obtaining one).
The admissions team will conduct an evaluation to determine eligibility and coordinate care (inpatient or home-based).
Services can often begin quickly once approved.
Description
Heart of Georgia Hospice provides compassionate end-of-life hospice care, including inpatient hospice, home hospice, pain and symptom management, emotional and spiritual support, and bereavement services for terminally ill patients and their families.
