Who Qualifies
Georgia resident, ages 18 to 64
Must have NO health insurance of any kind (no private, Medicare, Medicaid, or VA benefits)
Household income at or below 200% of the Federal Poverty Guidelines
Must live in Fayette, Coweta, or South Fulton County
Application Process

Bring a valid photo ID with your current address
Bring proof of residence (utility bill from the past month, or a notarized affidavit if living with someone else)
Bring proof of household income from the past 30 days (pay stub, Social Security statement, or a signed notarized affidavit if unemployed)
Eligibility is reviewed annually on July 1.
Description
Healing Bridge Clinic provides free primary health care to uninsured, low-income adults in Fayette, Coweta, and South Fulton counties. Services include general medicine, mental health counseling, women's wellness care, and specialty care such as cardiology, orthopedics, dermatology, and podiatry. Lab tests, X-rays, and imaging are also provided at no cost. Dental, vision, and physical therapy are available on a limited basis by referral.
