Application Process
There is no formal online application.
To apply:
Call the location you want to visit during office hours.
Tell them you need to make an appointment or register as a new patient.
Walk-ins may be accepted if the schedule allows, but calling ahead is strongly recommended.
Bring photo ID, proof of address, and proof of income for the sliding fee scale.
