Who Qualifies

• Have a child age 21 or younger with a physical disability or serious medical condition

• Demonstrate financial need, often including household income limits (commonly around $50,000 or lower)

• Provide documentation verifying the child’s diagnosis, requested need, and financial information

• Grants are usually one-time awards, often up to a few hundred dollars, and are paid directly to vendors or service providers.

Application Process

Visit the foundation website and download or complete the grant application.

Applications are reviewed periodically (often monthly) by the board.

Approved funds are issued directly to vendors or service providers supporting the child’s needs.

(Note: At times, the program may temporarily pause applications when funding cycles close.)
