Download and complete the Joinder Agreement (trust application) at gacommunitytrust.com
Mail your completed, notarized Joinder Agreement and one-time enrollment fee to GCT of BDI at 2120 Marietta Blvd NW, Atlanta, GA 30318 (call for fee amount)

Description
Georgia Community Trust (GCT) of BDI is a Pooled Special Needs Trust for Georgia residents with disabilities. It allows people to set aside money to pay for living expenses, equipment, transportation, and other needs -- without losing their Medicaid or SSI benefits. The trust manages and protects your funds, and you can request payments from your account whenever you need something your benefits do not cover.
