Primarily serves residents of Fayette County and surrounding areas.
Open to uninsured, underinsured, and low-income individuals and families.
Accepts new patients from the general public (no strict income or residency restrictions for most services, though priority is given to local residents).
Application Process

New patients can also walk in during open hours, but calling ahead is recommended.
Bring: Photo ID, proof of address, proof of income (for sliding fee scale), and insurance card (if you have one).
