Who Qualifies
Meet the following requirements:
Live in the United States or its designated territories.
Have had a medical appointment within the last 3 months.
Be under current care/supervision by a doctor for an active diagnosis of dysphagia (swallowing disorder).
Be under the supervision/care of a licensed Speech-Language Pathologist.
Have had a swallow study in the last three months.
Demonstrate financial hardship/low income. This is generally evaluated using your home state’s Medicaid eligibility guidelines.

Swallow study results (MBSS/FEES) or explanation
Proof of income (or extenuating circumstances statement)
Optionally: Doctor’s order/prescription for requested items

The Dysphagia Outreach Project Food Bank provides free access to thickeners, pre-modified foods, therapy tools, oral care supplies, and adaptive equipment to low income individuals with dysphagia.
