Who Qualifies:
You may qualify if you:

• Live in Georgia

• Are a U.S. citizen

• Have a traumatic brain injury or traumatic spinal cord injury

• Can show that other funding (like insurance, Medicaid, or Medicare) does not cover what you need

• You must provide medical records that explain your injury.

You will not qualify if:

• Your injury was not caused by trauma (for example, stroke, illness, or a condition you were born with)

• Your needs can already be paid for by insurance or another program

• You do not live in Georgia or are not a U.S. citizen

• Your application is missing required paperwork
